Name of Hospital, Clinic, or Person Providing Care
Diagnosis, Date of Treatment, Follow-up

With this information gathered if applicable you can get right through the health insurance application and get it processed as quickly as possible. Otherwise the insurance underwriters will stop the process until this information is obtained from the applicant.
